Attention athletes and parents! We are teamed up with Arbiter to stream line the physical process and athletic paperwork. This will be the only way athletic paperwork will be taken. 

Physicals must be dated after April 1, 2026 in order to be valid.  Please use all online forms when filling out information.

Three items should be uploaded.
1.  Doctor’s Physical Exam dated after April 1, 2026 with doctor signature

3.  Birth Certificate
